South Carolina Licensed Care Manager

Job Summary

A company is looking for a Concierge Care Manager to provide care management services to members.

Key Responsibilities

• Direct day-to-day high-touch member management and coordinate treatment plans for high-risk populations
• Coordinate clinical services with external providers, vendors, and community services
• Participate in team training and prepare for external audits and quality assurance efforts

Required Qualifications

• Associate's degree in Nursing or a Master's degree in Social Work, Psychology, or Counseling
• 5 years of case management experience as an RN or 2 years in a health insurance environment
• Active, unrestricted RN licensure or active licensure as a social worker, psychologist, or counselor
• Nationally recognized Case Management certification to be obtained within 2 years of hire
• Working knowledge of Microsoft Office software
