South Carolina Licensed Care Manager

Job Summary

Providing high-touch case management services, the full-time remote South Carolina Licensed Care Manager will administer complex care for specialized programs, coordinate healthcare services, and develop individualized care plans for members with high-risk conditions.

Key responsibilities

• Directs day-to-day member management, including treatment plan coordination and program goal implementation for high-risk populations
• Coordinates clinical services with external providers, vendors, and community resources
• Participates in team training and prepares for external audits and quality assurance efforts

Required qualifications

• Associate's degree in Nursing, or a Master's degree in Social Work, Psychology, or Counseling
• 5 years of case management experience or 2 years in a health insurance environment
• Active, unrestricted RN licensure or licensure as a social worker, psychologist, or counselor in the state of hire
• Nationally recognized Case Management certification to be obtained within 2 years of hire
• Working knowledge of Microsoft Office software
