Job Summary

A company is looking for a Case Management and Care Transitions Professional Development Specialist I.

Key Responsibilities

• Consult with leadership to develop, implement, and evaluate educational programs for Nursing and Social Workers
• Assist in the implementation of standardized components of education, including orientation and ongoing staff development
• Conduct annual competence assessments and performance improvement initiatives

Required Qualifications, Training, and Education

• Bachelor of Nursing degree preferred, with a minimum of one year of experience
• RN licensure required; enrollment in an accredited BSN program within two years for certain hires
• Prior leadership experience preferred
• One year of case management and/or utilization review experience preferred
• Case Management certification and a Master's Degree preferred
