Job Summary

A company is looking for a Chronic Disease Coach (RN or Dietician).

Key Responsibilities

• Assess member needs related to chronic conditions and coordinate personalized action plans
• Educate members and providers on chronic conditions and promote health program enrollment
• Maintain communication regarding member conditions and assist with referrals to support services

Required Qualifications, Training, and Education

• Associate's degree in a job-related field or equivalent experience
• 3 years of professional health and wellness experience
• Working knowledge of word processing software
• Ability to work independently and prioritize effectively
• Must obtain health coaching certification within 4 years of hire
