South Carolina Licensed Chronic Disease Coach

Job Summary

Providing personalized guidance and support, the full-time South Carolina Licensed Chronic Disease Coach will empower members to achieve optimal health through disease prevention and wellness promotion in a fully remote setting, with occasional on-site requirements for training and meetings.

Key responsibilities

• Assess members' chronic conditions and readiness for change, developing and implementing personalized action plans
• Educate members and providers on health conditions and available benefit plans, advocating for member needs
• Maintain compliance with regulatory guidelines while managing data collection and communication regarding member conditions

Required qualifications

• Associate's degree in a job-related field or equivalent experience (Graduate of an Accredited School of Nursing or 2 years of relevant work experience)
• 3 years of professional health and wellness experience
• Certification in health coaching must be obtained within 4 years of hire
• Working knowledge of word processing software and Microsoft Office
• Ability to work independently and prioritize effectively
