Job Summary
To oversee the coordination of healthcare services for patients with complex care needs, the full-time South Carolina Licensed Complex Care Coordinator will manage patient admissions, discharges, and transportation logistics while collaborating with various healthcare providers in a remote role with required travel.
Key responsibilities
- Coordinate healthcare services for specialty populations, ensuring safe and timely patient movement
- Manage transportation logistics and collaborate with outside agencies and legal counsel
- Build trusting relationships with patients and healthcare providers to facilitate seamless care transitions
Required qualifications
- Bachelor's degree in Nursing or Master's degree in Social Work from an accredited institution
- Licensure as a registered nurse or social worker by the state of South Carolina or compact state
- Three years of case management work experience preferred
- Certification in Case Management preferred
- Current Basic Life Support (BLS) certification from AHA or American Red Cross
