Job Summary

A company is looking for a Remote LPN Personal Care Coordinator.

Key Responsibilities

• Conduct monthly care management conversations via outbound phone calls to address patients' healthcare needs
• Communicate available resources and services to patients throughout their care continuum
• Maintain chart compliance and documentation, including medical history and care plan adjustments

Required Qualifications

• Current active, compact LPN/LVN license
• LPN/LVN degree from an accredited nursing program
• Ability to follow established clinical protocols and procedures
• Flexibility in task management and switching tasks effectively
• Willingness to assist co-workers and contribute to team success
