South Carolina Licensed Master Social Worker

Job Summary

A company is looking for a Licensed Master Social Worker (LMSW) in Primary Care.

Key Responsibilities

• Conduct patient contact and appointments to provide education, support, and resources
• Act as a healthcare and community resource liaison/advocate for patients and families
• Facilitate patient referrals to other healthcare providers and maintain relationships with resource providers

Required Qualifications and Education

• Master's degree in social work or social welfare from an accredited program
• MSW licensure by the South Carolina Board of Social Work Examiners
• Certification in case management preferred
• Experience in clinical and psychosocial assessments of mental health conditions
• Ability to work effectively in a team environment with healthcare providers and staff
