Job Summary

As part of the primary care treatment team, the full-time remote Licensed Master Social Worker (LMSW) will triage and manage select patients with medical and social needs, providing education, support, and resources while collaborating with healthcare providers and community resources.

Key responsibilities

• Conduct patient appointments to assess needs and provide support through direct contact and telephonic communication
• Act as a liaison between patients, primary care providers, and community resources to facilitate effective navigation and referrals
• Maintain relationships with healthcare and community resource providers, developing guides for services such as housing and financial assistance

Required qualifications

• Master's degree in social work or social welfare from an accredited program
• Current South Carolina Licensed Master Social Worker (LMSW) licensure
• Preferred certification in case management
• Experience working in a primary care setting or with healthcare providers
• Ability to perform clinical and psychosocial assessments of mental health conditions
