Job Summary
Working full-time in a remote capacity, the South Carolina Licensed Nurse Coordinator will facilitate healthcare delivery and promote communication among care teams for pediatric patients, ensuring appropriate care and follow-up services.
Key responsibilities
- Identify patients qualifying for care coordination and arrange necessary follow-up services
- Communicate effectively with patients, care teams, and providers to ensure continuity of care
- Assess patient needs and coordinate access to community resources to address barriers to care
Required qualifications
- Associate's Degree and a minimum of one year of experience as a Registered Nurse or three years as a Licensed Practical Nurse
- Current South Carolina RN or LPN license or compact state license
- Current Basic Life Support (BLS) certification from AHA or American Red Cross
- Experience in chronic care management for medically complex children is preferred
- Physical ability to perform job functions, including lifting and mobility requirements
