Job Summary

A company is looking for a Telehealth Preventative Primary Care Advanced Practice Provider- NP/PA.

Key Responsibilities

• Conduct primarily telehealth video visits with some home-based visits
• Perform comprehensive assessment visits including diagnosis, medication review, and quality of care assessments
• Coordinate care with the member's care team and utilize electronic health information systems

Required Qualifications, Training, and Education

• APRN with 3 or more years in outpatient clinical practice
• Experience in primary care, family medicine, geriatrics, and/or palliative care
• Experience with telehealth video platforms and Medicare annual wellness visits
• Active RN and APRN license in the relevant state and APRN certification required at hire
• Multi-state licensure preferred or willingness to obtain additional licensure
