Job Summary

To support a growing population health initiative, the part-time Remote Complex Care Nurse Practitioner (SC licensed) will provide telephonic and video-based follow-up care for patients post-hospital discharge, focusing on medication reconciliation, symptom screening, and care coordination across various settings.

Key responsibilities

• Cover care needs for the Transitions of Care program through telephonic outreach and scheduled visits
• Conduct medication reconciliation, symptom screenings, and order necessary follow-up services
• Collaborate with nursing teams and maintain accountability for individual performance targets while contributing to team goals

Required qualifications

• Certified Nurse Practitioner in Adult, Family, or Geriatrics with active SC licensure
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions
• Comfort in a digital-first environment with experience in virtual care delivery
• Bilingual in English and Spanish is a strong bonus
