Job Summary

To support a growing Population Health team, the part-time South Carolina Licensed Nurse Practitioner will provide comprehensive follow-up care via phone and video to patients post-hospital discharge, focusing on medication reconciliation and care coordination.

Key responsibilities

• Conduct telephonic outreach and scheduled video/phone visits for recently discharged patients
• Perform medication reconciliation, symptom screening, and order follow-up labs and medications
• Coordinate care across various settings and collaborate with the nursing team to achieve population health goals

Required qualifications

• Certified Nurse Practitioner in Adult, Family, or Geriatrics with active South Carolina state licensure
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• Direct experience managing complex, comorbid conditions in Medicare and Medicaid populations
• Comfort in a digital-first environment and familiarity with virtual care delivery
• Bilingual in English and Spanish, including cultural fluency, is a strong bonus
