Job Summary

To support a growing population health team, the part-time South Carolina Licensed Nurse Practitioner will provide telephonic and video-based follow-up care to patients post-hospital discharge, focusing on medication reconciliation, symptom screening, and care coordination.

Key responsibilities

• Manage telephonic outreach and scheduled visits for patients recently discharged from the hospital
• Coordinate care across various settings, including hospitals, pharmacies, and community providers
• Collaborate with nursing staff to achieve individual performance targets and contribute to population health goals

Required qualifications

• Active South Carolina Nurse Practitioner license
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• Experience engaging Medicare and Medicaid populations with complex conditions
• Motivational interviewing skills for patient engagement
• Comfort with virtual care delivery and digital tools
