South Carolina Licensed Population Health Manager

Job Summary

A company is looking for a Population Health and Manager of Post Acute Care Program Lead.

Key Responsibilities

• Develops, implements, evaluates, and maintains health management programs
• Coordinates care transitions for members from acute hospital care to post-acute settings
• Monitors utilization and evaluates member outcomes to ensure cost-effective care

Required Qualifications

• Bachelor's degree in a job-related field
• 5 years of clinical experience or experience in developing population health programs, with 2 years in a supervisory role
• Active, unrestricted RN licensure or equivalent licensure for Social Worker or Counselor
• Strong analytical and data interpretation skills
• Working knowledge of Microsoft Office software
