South Carolina Licensed Practical Nurse

Job Summary

To support chronic care management initiatives, the full-time South Carolina Licensed Practical Nurse will conduct outreach to patients with multiple chronic conditions, focusing on care coordination, patient education, and follow-up to enhance patient outcomes and reduce hospitalizations.

Key Responsibilities

• Conduct monthly telephonic outreach to assigned chronic care management patient panel
• Assess patient status and reinforce individualized care plans while documenting interactions in the electronic health record (EHR)
• Provide education on chronic disease management and support patient engagement in their health and treatment plans

Required Qualifications

• Active LPN licensure in good standing (South Carolina preferred)
• BLS (Basic Life Support) certification (if required by role)
• Prior experience in care coordination, case management, or chronic disease management preferred
• Experience working with high-risk or chronic condition populations strongly preferred
• Proficiency in EHR documentation and care coordination workflows, with experience in Epic Systems preferred
