South Carolina Licensed Practical Nurse

Job Summary

To support chronic care management initiatives, the full-time South Carolina Licensed Practical Nurse will conduct outreach to patients with chronic conditions, coordinate care, and promote patient engagement through education and follow-up, all in a remote setting.

Key Responsibilities

• Conduct monthly telephonic outreach to assigned CCM patient panel and document interactions in the electronic health record (EHR)
• Assess patient status, reinforce individualized care plans, and support medication adherence while identifying barriers to care
• Provide education on chronic disease management and encourage patient participation in their treatment plans

Required Qualifications

• Active LPN licensure in good standing, preferably in South Carolina
• BLS (Basic Life Support) certification, if required by role
• Prior experience in care coordination, case management, or chronic disease management preferred
• Experience working with high-risk or chronic condition populations strongly preferred
• Proficiency in EHR documentation and care coordination workflows, with experience in Epic Systems preferred
