South Carolina Licensed Practical Nurse

Job Summary

To enhance patient care, the full-time South Carolina Licensed Practical Nurse will conduct Chronic Care Management outreach to patients with chronic conditions, focusing on care coordination, patient education, and timely follow-up in a remote setting.

Key Responsibilities

• Conduct monthly telephonic outreach to assigned Chronic Care Management patient panel and document interactions in the electronic health record
• Assess patient status, support medication adherence, and identify barriers to care while collaborating with providers and care teams
• Provide education on chronic disease management and encourage patient engagement in their health and treatment plans

Required Qualifications

• Active LPN licensure in good standing (South Carolina preferred)
• BLS (Basic Life Support) certification (if required by role)
• Prior experience in care coordination, case management, or chronic disease management preferred
• Experience working with high-risk or chronic condition populations strongly preferred
• Proficiency in EHR documentation and care coordination workflows, with experience in Epic Systems preferred
