Job Summary

A company is looking for a Registered Nurse (RN) Ambulatory Care Manager - Population Health.

Key Responsibilities

• Provide clinical care management services to eligible patients to improve health outcomes and self-care abilities
• Develop and implement care plans, perform medication reviews, and ensure patient understanding of medical regimens
• Collaborate with healthcare providers and manage patient outreach and referrals to care and community resources

Required Qualifications

• Associate's Degree in Nursing; Bachelor's Degree in Nursing (BSN) preferred
• Active Registered Nurse license in the state of patient care
• 2-3 years of experience in acute care, home health, or case management
• Case Management certification preferred
• Strong analytical and data management skills
