Key Responsibilities

• Assess members' needs related to chronic conditions and implement personalized action plans
• Educate members and providers about chronic conditions and available health benefits
• Maintain compliance with regulatory guidelines and assist members with referrals to support services

Required Qualifications, Training, and Education

• Associate's degree in a job-related field or equivalent experience
• 3 years of professional health and wellness experience
• Knowledge of quality improvement processes and contract language
• Ability to work independently and prioritize effectively
• Must obtain health coaching certification within 4 years of hire
