Key Responsibilities

• Assess members' chronic conditions and develop personalized action plans to achieve health goals
• Educate members and providers about conditions and available benefit plans while advocating for members
• Maintain communication regarding member conditions and assist with referrals to appropriate support areas

Required Qualifications, Training, and Education

• Associate's degree in a job-related field or equivalent experience
• 3 years of professional health and wellness experience
• Working knowledge of word processing software
• Must obtain health coaching certification within 4 years of hire
• Ability to work independently and prioritize effectively
