South Carolina Licensed Social Worker

Job Summary

To support a thriving primary care team, the full-time South Carolina Licensed Social Worker will assist with discharge coordination, conduct patient assessments, and implement interventions to ensure successful transitions from hospital to outpatient care in a remote setting with occasional onsite meetings.

Key responsibilities

• Coordinate the discharge process in collaboration with the healthcare team to achieve targeted patient outcomes
• Conduct assessments of patients and caregivers during hospitalization and outpatient follow-up
• Identify risk factors post-discharge and apply interventions to minimize the likelihood of undesirable events

Required qualifications

• Master's degree in social work or social welfare from an accredited program
• South Carolina licensure as a Masters in Social Work (MSW) by the Board of Social Work Examiners
• Certification in Case Management preferred
• Experience in patient assessment and discharge planning
• Ability to work remotely with occasional onsite meetings in the greater Charleston area
