South Carolina RN Coordinator

Job Summary

To support comprehensive patient care, the full-time South Carolina RN Coordinator will deliver longitudinal care management services, focusing on chronic disease management and population health improvement while working remotely.

Key responsibilities

• Conduct ongoing telephonic and virtual outreach to engage an assigned patient panel
• Develop and update individualized care plans and patient-centered goals to support continuity of care
• Collaborate with interdisciplinary teams and community resources to enhance care coordination and patient outcomes

Required qualifications

• Active, unrestricted Registered Nurse (RN) licensure, preferably in South Carolina
• Minimum of 2 years of experience in care coordination, case management, or population health
• Experience with high-risk or chronic condition populations is strongly preferred
• Basic Life Support (BLS) certification, if required by the role
• Prior experience in program development or quality improvement is preferred
