Job Summary

A company is looking for a Telephonic Care Navigator - Remote in SC.

Key Responsibilities

• Conducts telephonic outreach to DSNP/CSNP members and educates them on care gaps
• Assists members with social determinants of health and connects them to community resources
• Maintains compliance with metrics, provides excellent customer service, and ensures confidentiality of health information

Required Qualifications

• High School Diploma / GED or equivalent work experience
• Must be 18 years of age or older
• 1+ years of call center and/or telephonic customer service experience
• 1+ years of professional experience in an office environment using telephone and computer
• 1+ years of healthcare/insurance or social work/community outreach experience
