Job Summary

Fluent in Spanish, the full-time remote Patient Success Representative will manage patient outreach, assist with technology support, and coordinate care to ensure a positive experience for families accessing mental health resources.

Key Responsibilities
  • Contact patients to schedule appointments, educate them on services, and follow up on missed appointments
  • Assist patients with setting up telehealth platforms and provide troubleshooting support for technology-related issues
  • Coordinate care by acting as a liaison between patients and providers, ensuring continuity of care and proper documentation
Required Qualifications
  • Bachelor of Social Work (BSW), Associate's degree in nursing (ADN), or relevant certification/education preferred
  • Minimum of 3 years of experience in a virtual patient-facing role, preferably with pediatric populations or young families
  • Proficiency in electronic health records (EHR) and basic technology troubleshooting
  • Ability to work independently and take initiative in a fast-paced environment
  • Commitment to health equity and reducing barriers to care
