Spanish Speaking RN Care Advocate

Job Summary

A company is looking for a Spanish Speaking Care Advocate Registered Nurse.

Key Responsibilities:

• Establish personal rapport with members to identify patient care requirements
• Resolve member problems and needs by coordinating with program staff
• Document all member phone calls and maintain consistency in records

Required Qualifications:

• Current, unrestricted Registered Nurse license
• Bachelors (or higher) degree in a health-related field preferred
• Minimum five years clinical experience in acute care setting or acute hospital discharge planning/transitions coordination
• Proficient in both English and Spanish
• Knowledge of medical management program planning, design, and implementation
