State Licensed APP Manager
Job Summary
Leading the Transitions of Care program, the full-time State Licensed APP Manager will provide clinical oversight and operational management for Advanced Practice Providers while delivering patient care remotely during standard business hours.
Key responsibilities
- Provide excellent patient care during the 30-day post-discharge period, managing symptomatic patients and reducing preventable readmissions
- Oversee clinical performance and accountability of the APP team, ensuring targets are met and identifying areas for improvement
- Conduct bi-weekly one-on-one meetings with direct reports, set personalized goals, and assist with recruitment and onboarding of new APPs
Required qualifications
- 5+ years of clinical experience as an Advanced Practice Provider (NP/PA)
- At least 1 year of experience managing post-discharge patients
- 2+ years of management or team leadership experience
- Active RN and APRN or Physician Assistant license with national certification
- Ability to work Monday through Friday from 9:30am to 6:00pm EST
