Job Summary

A company is looking for a Behavioral Health Care Navigator.

Key Responsibilities

• Support individuals with mental health conditions and substance use disorders by engaging them and assessing their needs
• Identify care goals and barriers to behavioral health care, providing navigation to appropriate services
• Collaborate with the clinical team on care issues and participate in case reviews

Required Qualifications

• 2+ years of experience as a Community Health Worker
• Active certification as a Community Health Worker
• Experience in a metric/quota-driven environment with a proven track record of exceeding performance indicators
• Demonstrated experience working with individuals facing complex chronic needs, mental health conditions, or substance use disorders
• Strong technical skills with CRM databases and omnichannel technology
