Job Summary
Supporting various teams remotely, the full-time State Licensed Care Coordinator will manage care coordination services for members, addressing inquiries, documenting interactions, and ensuring effective communication within the interdisciplinary care team.
Key responsibilities:
- Receive and respond to incoming calls from members regarding benefits, care coordination, and screening assessments
- Document calls accurately in the designated PHI system and manage an assigned member caseload
- Perform outreach to confirm services are in place and collaborate with the care team to address member needs
Required qualifications:
- Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training/experience
- High school diploma or GED
- Experience in a fast-paced environment handling multiple priorities
- Effective verbal and written communication skills
- Prior experience in a call center, care management, or healthcare environment is preferred
