Job Summary

A company is looking for a Director of Care Management.

Key Responsibilities

• Develop and execute the long-term strategy for care management services to optimize patient outcomes
• Drive transformation in care delivery by integrating clinical, behavioral, and social determinants of health
• Build and sustain external partnerships to enhance care continuity and address service gaps

Required Qualifications

• RN with specialization/experience in family medicine, internal medicine, and/or psychiatry
• 5+ years of experience as a Case/Care Manager with appropriate licenses and certifications
• 5+ years of experience leading a team
• 3+ years of experience with patients having complex chronic health conditions
• Experience in inpatient care settings preferred
