State Licensed Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop care/service plans in collaboration with healthcare teams to support member transitions
• Facilitate communication and services between various healthcare providers and resources

Required Qualifications

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
• 2-4 years of related experience in care management
• Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
• Preferred residence in Georgia, Tennessee, Nebraska, Oklahoma, or Washington
• Active RN license (compact license) is preferred
