State Licensed Care Manager

Job Summary

A company is looking for a Care Manager to coordinate and oversee behavioral health services for members.

Key Responsibilities

• Conduct holistic assessments, develop individualized care plans, and coordinate care transitions and crisis prevention plans
• Build trusting relationships with members and providers to enhance engagement and continuity of care
• Ensure compliance with clinical, regulatory, and organizational standards through accurate documentation and participation in quality improvement initiatives

Required Qualifications

• Current, unrestricted state license as a Clinical Social Worker, Marriage and Family Therapist, Professional Counselor, Clinical Psychologist, or Registered Nurse
• 3-5 years of direct clinical care experience in behavioral health or substance use treatment settings
• Strong clinical judgment and case management skills
• Ability to interpret and apply medical policy and clinical criteria
• Proven interest in professional development through specialization or advanced degrees
