Job Summary

A company is looking for a Case Manager.

Key Responsibilities:

• Create and promote adherence to treatment plans developed by healthcare providers
• Monitor and document patient progress and maintain accurate clinical reports
• Connect patients to community resources and manage relationships with care providers

Required Qualifications:

• Current standing as a registered nurse (RN) or licensed practical nurse (LPN)
• 5 years of experience in a clinical or community resource setting/care coordination
• 2 years of case management experience
• Highly organized with the ability to maintain accurate records
• Proficient computing skills and ability to learn new systems
