Job Summary

A company is looking for a Case Manager who will act as a liaison between patients and healthcare providers.

Key Responsibilities

• Assess, plan, implement, coordinate, monitor, and evaluate patient care options
• Serve as the primary contact between patients, primary care physicians, and other healthcare providers
• Ensure timely completion of tasks while managing workload based on client needs

Required Qualifications

• Associate's Degree in Nursing
• Active RN License required
• Minimum three years of clinical care experience
• Active CCM Certification or ability to obtain within 18 months of hire
• Bachelor of Science in Nursing preferred
