Job Summary

A company is looking for a Manager, Clinical Care Case Management - Remote.

Key Responsibilities

• Manage day-to-day operations of Health Plan Clinical Programs to improve clinical efficiencies
• Oversee case management for high-risk patients and ensure smooth transitional care management
• Implement and direct health education activities for beneficiaries and providers

Required Qualifications

• RN/BSN required
• 5 years of management experience
• 4 years of clinical nursing experience
• At least 4 years of experience in chronic care case management in an outpatient setting
• Certification in case management preferred
