State Licensed Clinical Care Specialist

Job Summary

A company is looking for a Clinical Care Specialist to manage chronic care for patients remotely.

Key Responsibilities

• Manage a patient census and coordinate Chronic Care Management (CCM) services for chronically ill patients
• Create individualized Care Plans and monitor labs, vitals, medications, and testing
• Conduct care coordination and provide education on chronic condition management to patients and their families

Required Qualifications

• Registered Nurse (RN) licensure in applicable state
• Five years of nursing experience
• Experience with chronically ill patients preferred
• Telephonic case management or care coordination experience preferred
• Self-motivated and able to work independently
