Job Summary

A company is looking for a Clinical Pharmacist to provide essential pharmacy services that contribute to optimal patient outcomes.

Key Responsibilities

• Oversee therapy management and address medication errors and adverse reactions
• Participate in quality improvement initiatives and ensure adherence to established standards
• Support medication use processes and collaborate with interdisciplinary teams to enhance patient care

Required Qualifications

• Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy
• Current Pharmacist license from the appropriate state licensing authority within 2 months of hire
• American Heart Association BLS certification within 6 months of hire
• Preferred: Completion of an ASHP accredited PGY1 or specialty residency
• Preferred: 3 years of retail or specialty pharmacy experience
