Job Summary
State Licensed Clinical Pharmacist, responsible for developing and implementing medication therapy management programs, conducting medication reviews, and collaborating with clinical staff in a full-time remote position.
Key Responsibilities
- Perform and document comprehensive medication reviews for members
- Drive medication interventions to ensure adherence to evidence-based guidelines
- Collaborate with clinical teams and participate in interdisciplinary care meetings
Required Qualifications, Training, and Education
- Bachelor's degree in pharmacy or PharmD
- Active pharmacist license in the state of residence
- 3+ years of experience as a pharmacist, preferably in managed care or clinical settings
- PGY1 residency in ambulatory care or related clinical field preferred
- Board certification in pharmacotherapy or MTM is desirable
