Job Summary
To support enterprise clinical programs, the full-time remote State Licensed Clinical Pharmacist will manage patient engagement, medication optimization, and clinical decision support initiatives while collaborating with medical leadership and care teams to improve quality and outcomes.
Key responsibilities
- Operate within pharmacist-led clinical programs focused on chronic disease management and medication optimization
- Conduct Medication Reconciliation, Medication Therapy Management (MTM), and Comprehensive Medication Reviews (CMR) while providing telephonic patient education
- Collaborate with cross-functional teams to develop patient-specific clinical action plans and drive the adoption of actionable drug-related recommendations
Required qualifications
- Doctor of Pharmacy (PharmD)
- Active pharmacist license in good standing (state-specific: CA, WA, KS, OH, NY, NJ, FL, MD, NH, MS, LA)
- Experience in Medication Therapy Management and pharmacist-led clinical programs
- Strong understanding of evidence-based guidelines and quality measures
- Experience or interest in value-based care or population health environments
