Job Summary
To support a growing healthcare initiative, the full-time State Licensed Disease Management Coordinator will collaborate with patients and primary care providers to ensure quality healthcare services, while managing care coordination and monitoring patient outcomes in a remote setting.
Key responsibilities
- Coordinates, monitors, and evaluates healthcare options and services to optimize patient health status
- Identifies targeted populations, risk stratifies patients, and directs interventions accordingly
- Collaborates with multidisciplinary teams to implement discharge plans and ensure safe transitions of care
Required qualifications
- Associate's Degree in Nursing with seven years of clinical experience or Bachelor's Degree in Nursing with five years of clinical experience
- Current Registered Nurse license issued by the state or multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC)
- Certification in Basic Life Support within 30 days of hire date
- Experience in care coordination preferred
- State and federal criminal background checks as required for regulated areas
