Job Summary

A company is looking for a Health Coach - Clinical Navigator.

Key Responsibilities

• Provide one-on-one health coaching for patients managing chronic conditions
• Utilize motivational interviewing and behavioral change techniques to support patient health improvement
• Educate patients on disease management and offer ongoing support through remote monitoring tools

Required Qualifications

• Active health coaching certification (NBHWC, NSHC, ACE Advanced)
• 1+ years of experience in health coaching or patient support, particularly in chronic condition management
• Proven expertise in behavior change techniques and motivational interviewing
• Experience with remote patient care or telehealth is highly desirable
• Strong organizational skills and ability to work independently in a virtual environment
