Job Summary

Supporting members in their recovery journey, the full-time Remote Transitions of Care Associate will advocate for patients post-discharge, coordinating necessary resources and ensuring they receive appropriate care to prevent hospital readmissions.

Key responsibilities

• Conducting inbound and outbound calls to check on patients after discharge and confirm they have the needed assistance
• Coordinating services with affiliated vendors, including transportation, home health care, and meal delivery
• Collaborating with patients, providers, and caregivers to facilitate positive health outcomes and scheduling follow-up visits

Required qualifications

• Active and clear Medical Assistant Certification
• 2-3 years of experience in healthcare, managed care, home health, or community health settings
