Job Summary
Providing patient-centered MTM services, the full-time State Licensed MTM Pharmacist will optimize medication therapy through comprehensive reviews and proactive patient outreach while working remotely.
Key responsibilities
- Deliver MTM services, including Comprehensive Medication Reviews and Adherence Monitoring Interventions
- Conduct proactive patient outreach to explain MTM program benefits and engage hesitant patients
- Communicate clinical recommendations to prescribers and document all patient interactions accurately
Required qualifications
- Bachelor's Degree in Pharmacy or PharmD from an accredited college
- Current license to practice Pharmacy in the applicable state(s)
- Completion of an MTM certificate course
- Minimum of 1 year of relevant work experience
- Ability to acquire pharmacist licensure in multiple states if requested
