Job Summary

A company is looking for a Remote Nurse Care Coordinator.

Key Responsibilities

• Assess and analyze the injured worker's medical, behavioral, and vocational status
• Develop and implement Care Coordination plans for patient functional improvement
• Manage patient progress and arrange referrals and consultations as needed

Required Qualifications

• Current, active, and unrestricted Nursing License (RN or LPN)
• Preferred 1-2 years of Workers' Compensation and Case Management experience
• Certified Case Manager (CCM) designation preferred
• Bilingual ability in English and Spanish required
• Availability to work 11:30am-8:00pm EST
