State Licensed Nurse Care Manager

Job Summary

A company is looking for a Nurse Care Manager for their Compassionate Care Program.

Key Responsibilities

• Build empathetic relationships with patients and families while providing education on diagnosis and treatment options
• Conduct assessments and advance care planning to identify patient needs and develop collaborative care plans
• Coordinate care across multiple providers and ensure accurate record-keeping of patient care

Required Qualifications

• Current RN License required; BSN preferred
• 1+ years of utilization management experience preferred
• 3+ years of clinical practice in an ICU, hospice, or palliative care role
• Experience with goals of care discussions and end-of-life care decisions required
• Intermediate skills with MS Office Suite and ability to work effectively in a remote environment
