State Licensed Nurse Care Manager
Job Summary
Supporting the Evergreen Aligned team, the full-time remote State Licensed Nurse Care Manager will enhance clinical outcomes and patient engagement by executing individualized care plans for patients with chronic kidney disease, hypertension, diabetes, and congestive heart failure.
Key responsibilities
- Conduct pre-visit planning and coordinate necessary clinical information for scheduled visits
- Reinforce care plans through follow-up and ensure patient understanding and adherence
- Provide education and support for chronic disease management, including CKD, hypertension, and diabetes
Required qualifications
- Active, unrestricted Registered Nurse (RN) license
- 2+ years of clinical experience in chronic disease management (CKD, diabetes, or hypertension)
- Experience with electronic health records (EHR)
- Ability to manage multiple patients in a fast-paced virtual environment
- Strong organizational and follow-through skills
