Job Summary

A company is looking for a Clinical Navigation, Nurse Practitioner (Per Diem).

Key Responsibilities:

• Assess patients' needs, gather clinical history, and provide guidance to navigate healthcare resources
• Deliver high-quality clinical services and monitor care quality to meet patient satisfaction benchmarks
• Evaluate and manage patients with acute, chronic, and mental health conditions while collaborating on efficiency improvements

Required Qualifications:

• Licensed NP/PA provider in good standing with state board
• Maintains board certification
• At least 3 years of NP/PA experience in a clinical setting
• Comfortable providing care to all age groups
• Ability to thrive in a dynamic work environment while working independently
