Job Summary

Providing virtual primary care through an innovative telehealth platform, the full-time State Licensed Nurse Practitioner will conduct comprehensive clinical assessments, manage chronic conditions, and emphasize preventative care while collaborating with a multidisciplinary team remotely.

Key responsibilities

• Conduct virtual primary care visits, including comprehensive assessments and chronic disease management
• Diagnose and manage chronic conditions, developing individualized care plans to optimize patient health
• Coordinate follow-up care, referrals, and health coaching to empower patients in managing their health

Required qualifications

• Active Advanced Practice Provider (APRN or PA) license in good standing in specified states
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine
• Minimum of 2 years of primary care, urgent care, or internal medicine experience, preferably in telehealth
• Experience managing chronic conditions and preventive care via telehealth
• Active BLS certification at the time of hire
