Job Summary
Offering a remote opportunity, the State Licensed Oncology Social Worker will provide comprehensive psychosocial support to patients and their families during their cancer journey, conducting assessments, linking resources, and coordinating care with healthcare teams.
Key responsibilities
- Conduct comprehensive psychosocial assessments to identify and address emotional and social needs of patients and families
- Identify and connect patients with relevant community resources to address complex social needs
- Collaborate with healthcare teams to ensure coordinated care and advocate for patients' needs with insurance and healthcare providers
Required qualifications
- Master's degree in Social Work from an accredited program
- Valid and current master's level license in the current practice state (e.g., LMSW, LISW, LCSW)
- Preferred certifications include OSW-C or APHSW-C
- Minimum three to five years of experience as a social worker in a healthcare setting, with oncology or palliative care experience desired
- Willingness to obtain additional state licenses as needed
