Job Summary

A company is looking for a Part-Time Pharmacist responsible for managing medication programs for kidney disease and related comorbidities.

Key Responsibilities

• Review and adjust medication regimens based on protocols and guidelines
• Create educational materials regarding drugs and treatment updates for providers and staff
• Collaborate on treatment algorithms and evaluate high-cost or high-risk medications

Required Qualifications

• PharmD degree from an accredited School of Pharmacy with ASHP Accredited Residency
• 3 years of clinical pharmacy experience in direct interaction with healthcare providers
• Active Pharmacy License
• Experience in renal patient pharmacy management and value-based care preferred
• Understanding of CMS Star measures, HEDIS measures, MIPS, and National Quality Forum measures
